Information for Patients

New Patients

Welcome! We look forward to meeting you! Please take a moment to review the following information for new patients as well as our general office policies. 

  • You will receive a link to complete your self check-in prior to your appointment.  

    • Please complete the self check-in process on the patient portal prior to your appointment. This will allow us time to review your history in advance and make the most of our time together to address your current healthcare needs. 

    • You will receive the consent for treatment, financial and privacy policies in your patient portal when you schedule an appointment and at least annually thereafter. Please make sure to sign them prior to your appointment. 

  • In order to make our time together as helpful as possible for you, you may want to consider making a few notes about your current concerns, care you’ve received for those concerns in the past, questions, and goals for our work together. 

  • Please bring the following to your appointment:

    • Driver’s license

    • Insurance card (we may need this for billing labs)

    • Method of payment 

    • List of medications

    • Awareness of personal and family medical history if available 

    • Relevant records or labs from outside providers (see below)

  • Labs may be an important part of your care. We know many patients will come to us having had labs done with another provider in the recent past. In order to avoid unnecessarily repeating labs, please bring either printed copies of recent labs or know how to locate them on your phone through your provider’s patient portal. 

  • If you haven’t had recent labs done, no problem! We are happy to order them as needed. If you have private insurance, we are able to bill that insurance directly for lab work. Please find out in advance if your insurance requires the use of a specific lab.

  • If you have medical records from an outside provider that you think will be relevant to our work together, you can send them to us through the patient portal or you can request the outside provider share your records directly with us.

Returning Patients

  • Please bring the following to every appointment:

    • Driver’s license

    • Insurance card (for billing for labs if needed)

    • Method of payment

    • Updated medication list

    • Relevant records or labs from outside providers. If you have lab results available on your phone, please familiarize yourself with how to locate them prior to your visit. 

  • All patients are required to update their acknowledgment of reading/receiving the consent for treatment, financial and privacy policies once a year. If your consent forms are expired you will be asked to sign them again. In addition, if our policies or forms change, you may be required to acknowledge and sign the new form.
